By registering for My Health Online you will be able to:
- Book and Cancel Appointments Online
- Order Repeat Medication
Please note: If you have recently transferred to us from Cae Teg Health Centre and were previously registered for My Health Online, you will need to reregister with us. We use different clinical systems and therefore your My Health Online access is unable to transfer.
My Health Online (MHOL) Registration during COVID-19
In normal circumstances when patient’s register for using MHOL this is done in a face to face setting where copy identification documentation is viewed prior to provision of a MHOL access being granted.
During Covid-19 there is more demand for online services and access with these being performed remotely via telephone and or email solutions, rather than in face to face settings.
Most patients will be able to prove their identity with identification documents such as a passport or driving license. Therefore we request that a patient completes the MHOL request form electronically and email this to us along with:
- a photo (or scanned copy) of your passport photo or photo driving licence
- plus an additional form of proof of address, for example, a copy of your recent utility bill, where you have technology available.

We would like to remind our patients to take account of their own security. Sending information by email does not provide end to end encryption and is done so at the individuals own risk.
Alternatively, patients can hand deliver the form and the copy documentation to the surgery.
We will aim to return to you via email within 72 working hours with confirmation of your new My Health Online username and password. Alternatively, these details could be made available for collection from the surgery.
